Our Practice Philosophy

Communication and Education

Becoming a Patient in our practice

    Access the Patient Portal to request a consultation. You do not need to be an established patient to use this service.

    Download and fill out the following forms:

    Collect your medical records and bring to your appointment.

    Bring a list of your currently prescribed medications to your appointment.

    Come to your office visit fasting 14 hours with no caffine for 24 hours and in clothes appropriate for excercise in case a lipid panel or stress test is necessary.

    Bring a list of your other health care providers so that we may coordinate your healthcare with them.

    Review our Health Bank to learn about your medical concern and make a list of questions to address at your visit.
